This FREE (2-Session- 2-Hour) Virtual Group Program is a structured, interactive education and self-management program for individuals who are newly diagnosed with Type 2 Diabetes or who could benefit from additional support in managing their diabetes.
Program Goal: To empower individuals with Type 2 Diabetes with the knowledge, skills and confidence needed to actively participate in their diabetes management, support healthy lifestyle behaviours and reduce the risk of diabetes-related complications.
This program, delivered over two sessions, uses facilitated conversation and practical activities to help participants build knowledge, confidence, and skills related to diabetes self-management. It emphasizes patient-centered learning, goal setting, practical strategies that participants can incorporate into their daily lives.
Topics include:
Understanding Type 2 Diabetes and its effect on the body;
Healthy Eating
Physical Activity
Blood Glucose Monitoring
Medications
Lifestyle Modification; and
Strategies for Reducing the Risk of Diabetes-Related Complications
A Follow-up Questionnaire will be used to assess participants’ understanding, confidence, and application of diabetes self-management strategies following completion of program.
