Looking for a family physician?

We are accepting new patients! The Scarborough Academic Family Health Team (SAFHT) is pleased to offer a patient waitlist for individuals and families who are looking for a family physician.

Our affiliated doctors are accepting new patients as capacity becomes available. Because we receive a high volume of applications, there may be a significant wait before we are able to review your application and identify a suitable physician.

We appreciate your patience while we work to connect patients with available doctors in the communities we serve.

How the waitlist works

When you submit an application, the information you provide will be reviewed by our team to determine whether we can match you with an affiliated doctor.

We will consider factors such as:

· Your geographic location and postal code

· The availability and capacity of our affiliated doctors

· The location where an affiliated doctor provides care

Our goal is to match patients with an affiliated physician who serves their geographic area and has capacity to accept new patients.

Submitting an application does not guarantee you will be accepted as a patient nor does it provide a specific timeline when this will happen. Availability can change and wait times may vary depending on physician capacity and the number of applications we receive.

If we are able to identify a potential match, a member of our team will contact you using the contact information you provide. Please ensure we can reach you at your stated telephone number and email address.

Before you apply

Please consider the following:

If you already have a family doctor:

You should generally remain connected to your current primary care provider while waiting for a new doctor. Joining our waitlist does not require you to leave your current provider.

If you are already registered with Health Care Connect:

You may remain registered with Health Care Connect while applying through our waitlist. SAFHT's waitlist is separate from the provincial Health Care Connect program. You can learn more about Health Care Connect through the Government of Ontario, by clicking here.

If your contact information changes:

Please notify us if your telephone number, email address, home address or other relevant information changes. Outdated information may make it difficult for us to contact you when an opportunity becomes available.

What information do we collect?

The application asks for information that helps us determine whether and where you may be matched with an affiliated physician. This may include:

· Your name and contact information

· Year of birth and other identifying information

· Postal code

· Information about your current family doctor if applicable

Please provide only information that is requested on the application form.

How will my information be used?

The information you provide will be used to:

· Process and manage your patient waitlist application

· Determine whether you may be eligible for attachment to an affiliated physician

· Match you with an affiliated doctor based on geographic location, physician availability and the information you provide

· Contact you regarding your application or a potential doctor match

· Maintain and manage our waitlist

· Support administrative, quality improvement and planning activities

Your information will not be used to provide you with medical care simply because you have submitted a waitlist application. You will become a patient of an affiliated doctor only if you are successfully matched and accepted for ongoing care.

Your privacy is important to us

SAFHT is committed to protecting the privacy and confidentiality of your personal and health information.

Information collected through the waitlist application will be handled in accordance with applicable Ontario privacy legislation and SAFHT's privacy practices. Access to your information is limited to individuals who require it to perform their duties.

We take reasonable safeguards to protect information against unauthorized access, collection, use, disclosure, loss or theft.

You have rights regarding your personal health information, including the right to request access to information we hold about you and, in most circumstances, request that inaccurate information be corrected.

Important information about the waitlist

The waitlist is not an urgent-care service.

Submitting an application does not provide access to a family physician while you are waiting, and SAFHT cannot provide medical advice or treatment through the waitlist.

If you have an urgent health concern, please seek appropriate medical care through your existing health care provider, an urgent care service, emergency department or other appropriate health service.

The form will take approximately 5 minutes to complete. A separate form is required to be completed for each individual seeking care including children, spouses, and other family members.
