Finding a Doctor

You become a patient of the Scarborough Academic Family Health Team (SAFHT) by first becoming an enrolled patient of a doctor on the team. You can't enrol directly with the SAFHT.

If you do not already have a doctor please look at the Doctors office location section. This section provides the addresses, maps, office hours and phone numbers of the family physicians on our team. You can contact the office directly and ask if the doctor is still taking on new patients.

The preferred method to finding a physician is to contact Health Care Connect at 1-800-445-1822, OR you can go on-line at http://www.health.gov.on.ca/en/ms/healthcareconnect/public/default.aspx to register for the unattached patient database. Health Care Connect is a new program that has been created by the Ministry of Health and Long Term Care to identify and place unattached patients across the province of Ontario with health care providers.

Health Care Connect will refer people without a family health care provider to physicians and nurse practitioners who are accepting new patients within their communities. After an individual has registered with Health Care Connect, a Registered Nurse called a Care Connector will be assigned to each unattached patient to assist them in finding a health care provider in their area. All individuals registering with Health Care Connect must have a valid OHIP card to participate in the program. Please note that you must bring a valid Ontario Health card when you enrol, and for every appointment.

When you enrol with SAFHT our Team is committed to providing you with high quality and comprehensive health care. If you wish to change your primary care provider, please speak with them as soon as possible.

FHO After Hours Clinic


The After Hours Clinic is a walk-in-clinic for enrolled patients of doctors affiliated with the Scarborough Academic Family Health Team. There are two clinics operating for two different sets of doctors

If you require medical care after hours or on weekends, your Doctor’s office voice mail will provide you with a phone number of the Doctor who is on call, or for the location of this week’s after hours clinic.

Please note that you must bring a valid Ontario Health Card to your appointment.

Important: If you have a medical emergency please go to your nearest emergency department or call 911.

Telephone Health Advisory Service:

You may also speak to a nurse any time after hours by calling the Telephone Health Advisory Service at 1-866-553-7205. The service is open – Weekdays 5:00pm – 9:00pm and Weekends and Recognized Holidays: 24 hours.

How to Enrol as a Patient

You must bring a valid Ontario Health card when you enrol.

The Physician will ask you to complete the Patient Enrolment and Consent to Release Personal Health Information form (the enrolment/consent form). Once the form is completed, it is sent to the Ministry of Health and Long-Term Care for processing.

By completing and signing the Patient Enrolment and Consent to Release Personal Health Information form, patients commit to the following:

  • To seek treatment from their family doctor or the group first, unless they are travelling or find themselves in an emergency situation;

  • To allow the Ministry to provide their doctor or the Family Health Team with information about services they have received from family doctors outside the FHT, and some preventive care services;

  • To not switch the doctor or Family Health Team they are enrolled with more than twice per year.

Patients remain enrolled unless they are no longer eligible for OHIP, move outside of the geographic area of the Family Health Team, enrol with a primary health care physician in another group, or choose to cancel their enrolment.

Physicians may choose to discontinue acting as a patient's family doctor in accordance with the College of Physicians and Surgeons of Ontario (CPSO) guidelines. Enrolment may also end when: the physician to whom the patient is enrolled leaves the Family Health Team; the patient consistently fails to meet the obligations set out in the Patient Commitment on the enrolment form; or when the patient becomes a resident of a nursing home or chronic care facility.

Enrolling with SAFHT ensures that you have access to primary health care treatment or advice, 24 hours a day, seven days a week, through a combination of regular and extended office hours, and a nurse-staffed Telephone Health Advisory Service (THAS).

Scheduling Appointments

Contact the Doctor's office directly to set up an appointment. Please note that you must bring a valid Ontario Health card when you register, and for every appointment. Please bring your health card and a list of your current prescriptions and over-the-counter medications that you are taking to the appointment. If you cannot make your appointment we require at least 24 hour notice so your appointment can be offered to someone else.

Please speak to your doctor, or nurse about a referral to SAFHT's FREE services that are noted under the tab for Programs and Services.

Important: SAFHT's family physicians ensure a continuity of care for you in a shared care environment. This means that clinicians at SAFHT consult and share information on the assessment, diagnosis, treatment and ongoing monitoring of patients and their disease conditions. In most instances this data is part of a patient's electronic medical record retained by the doctor.
